REGULATION 4   CONTRIBUTED AMOUNTS PAID STATEMENT - ADDITIONAL INFORMATION (ACT, S 12(3))  

4(1)   [Schedule 3 matters]  

A contributed amounts paid statement must include information (if applicable) on the matters set out in Schedule 3 .

4(2)   [Matters to be included]  

Those matters include:


(a) matters required by paragraphs 12(3)(c), (d) and (e) of the Act to be set out in the statement (which are marked with an asterisk* in the Schedule); and


(b) matters that, under paragraph 12(3)(f) of the Act, are required by these Regulations to be so set out (which are unmarked).
