What you can do with this form

Use this form if you’re under a review or audit and you want to tell us about:

  • any mistakes you have made
  • anything you have left out of your tax returns or statements.

If we inform you that we are going to conduct an examination of a particular accounting period, you must use this form to disclose errors.

Give the completed form to the tax officer conducting the review or audit.

Help with completing the form

The voluntary disclosure must be full and complete to the best of your knowledge or ability.

You should follow the instructions to help you complete the following fields in the form:

  • Section A: Your details
  • Section B: Return of statement details.
    • For questions 10 and 11, include as much detail as you can so we can assess your disclosure in a timely manner without having to ask for more detailed information.
    • For question 13, note any documents to support your voluntary disclosure, we may ask you to provide these if we need them.
  • Section C: Declaration.

Keep a copy of this form with any attachments and supporting documents for your own records.
