We have pre-filled your tax return with private health insurance policy details provided to us. Check for additional policies you held that are not pre-filled and ensure you enter them.

Here are details about changes to private health insurance.
You will need a statement from your registered health insurer. If you did not receive a statement, contact your insurer. Your tax return may be delayed if you do not use your statement and the private health insurance details you provide are incorrect.
Dependent children
If you were covered as a dependent child on a family policy, use the information shown on the statement for that policy.
Whether a person is a dependent child is determined by the rules of your health fund. If you are under 25 years old and do not have your own policy, you may be covered as a dependent child on your family policy. If you are unsure about whether this applies to you, check your family private health insurance policy details or contact your insurer.
Completing your tax return
Claiming for yourself only
- Select the Add policy button.
- Select the Health insurer ID from the drop-down menu, shown at B on your statement.
- Enter your Membership number, shown at C on your statement.
- Select your Tax claim code from the drop-down menu.
On 30th June 2015 if you:
- were single and had no dependants, select code A
- were single and had a dependent child (your child or a sibling who is dependent on you for economic support), or you paid for a dependent-child-only policy, select code B
- had a spouse (including if your spouse died during 2014–15 and you did not have another spouse before 30 June 2015), select code C.
- Enter Your premiums eligible for Australian Government rebate, shown at J on your statement. Do not show a percentage.
- Enter Your Australian Government rebate received, shown at K on your statement. Do not show a percentage.
- Select the Benefit code from the drop-down menu, shown at L on your statement.
- If you have more than one line on your statement repeat steps 4 to 7 for each line.
- Select the Save button.
Claiming for yourself and your spouse's share of the rebate
You can choose to claim your spouse's share of the rebate on their behalf if both of you meet all of the following three criteria. Both of you:
- were covered under the same policy
- were covered for the same period of time
- were together on 30 June 2015.
Firstly, enter the details for yourself:
- Select the Add policy button.
- Select the Health insurer ID from the drop-down menu, shown at B on your statement.
- Enter your Membership number, shown at C on your statement.
- Select Tax claim code C (for yourself) from the drop-down menu.
- Enter Your premiums eligible for Australian Government rebate, shown at J on your statement. Do not show a percentage.
- Enter Your Australian Government rebate received, shown at K on your statement. Do not show a percentage.
- Select the Benefit code from the drop-down menu, shown at L on your statement.
- If you have more than one line on your statement repeat steps 4 to 7 for each line.
- Select Yes to the question ‘Do you want to claim your spouse’s share?’.
- Your spouse’s share will be automatically completed with tax claim code D. Select the Save button.
Your spouse is claiming your share of the rebate
- Select the Add policy button.
- Select the Health insurer ID from the drop-down menu, shown at B on your statement.
- Enter your Membership number, shown at C on your statement.
- Select Tax claim code E from the drop-down menu. You do not need to enter any other information into the table.
- Select the Save button.
Information you enter if you are a dependent child
- Select the Add policy button.
- Select the Health insurer ID from the drop-down menu, shown at B on your statement.
- Enter your Membership number, shown at C on your statement.
- Select Tax claim code F from the drop-down menu. You do not need to enter any other information as a dependent child cannot claim the rebate.
- If you have more than one line on your statement and your circumstances changed during the year, select the correct tax claim code that applies for each line.
- Select the Save button.
