REGULATION 4A APPLICATION FOR REGISTRATION FOR ENTITLEMENT TO PRODUCT STEWARDSHIP (OIL) BENEFITS (ACT s 9)

For subsection 9(1) of the Act, an application for registration for entitlement to the product stewardship (oil) benefits must include:

(a) the applicant's name, business address, postal address, telephone number and fax number (if any); and

(b) the applicant's ABN; and

(c) the applicant's licence number under section 34 of the Excise Act 1901 .
