Private Health Insurance Act 2007
A statement, notice, application or other document is in the approved form if: (a) it is in the form approved in writing by the person specified in the table as the approver of that form; and (b) it contains a declaration signed by a person or persons as the form requires (see section 333-15 ); and (c) it contains the information that the form requires, and any further information, statement or document required by the approver, whether in the form or otherwise; and (d) it is given in the manner (if any) required by the approver.
