SECTION 91H (Repealed by 101 of 2006) APPLICATION FOR ACCREDITATION 91H(1) [Approved form] An application for accreditation must be in a form approved in writing by the Commissioner for the purpose and must contain the information necessary for the proper completion of the form. 91H(2) Electronic applications.
